The story of haves and have-nots in a world in which there’s almost nothing left to have. 200 years after the apocalypse, the gentle denizens of luxury fallout shelters are forced to return to the irradiated hellscape their ancestors left behind — and are shocked to discover an incredibly complex, gleefully weird, and highly violent universe waiting for them.
This tv show contains 95 potentially triggering events.
Not in the way I assume you mean. But the almost the whole first season The Ghoul is hunting a bounty on an object Lucy is harboring. So he does follow her and she is on the run.
(Ep. 3 - 4) A major character is physically abused and threatened within a military organization. He's later given command over one of his former bullies, and abuses his power over the bully in exactly the same way his superiors did with him. However, it's only for a short time and he changes his behaviour in later episodes.
There is discussion of experimentation on children, however it is never shown, also many of the vaults experiment on people and that probably includes children.
S1E2.. companion (dog) is stabbed by The Ghoul in Filly. It's not seen but you do hear it. It's when the dog attacks The Ghoul. You do later see the dog and its stab wound a couple of times in passing. Later The Ghoul heals the pup back to life.
Spoilers S01E01 Rape by Fraud. A man pretends to have been selected to marry a woman for breeding purposes. He's actually an irradiated raider who tries to kill his new "wife" after they have unprotected sex
Though technically not a dislocation, character's body parts are often ripped or cut off, including Cooper and Lucy's fingers. A character keeps a drawer full of amputated fingers.
Episode 2 from 22:10 to 23:15 a man coughs and says that he can't breathe, he dies off screen.

There are creatures known as "Ghouls" in the Fallout universe, who are old humans who have been exposed to extreme radiation. They have no noses, and their skin is very burnt.
In episode 2 a man has his foot shot off then “replaced” with a horrendous mechanical replacement that takes off more of his leg and makes him bleed profusely throughout the rest of the episode
In episode 6 the main character watches a video from a medical experiment in which a pregnant woman delivers piranha-like critters who turn around and eat her below the waist
Sequence starts around 50
In episode 4 around the 13-14 minute mark the main character bites off the ghouls finger and he subsequently cuts off hers. It’s done very slow and in a very graphic manner.
Not sure why the “no” answers. A main character is weighed down by anchors and repeatedly submerged in a lake. Afterwards, she is briefly put on a leash and is later forced to do degrading things such as drinking out of a bucket of irradiated water. She later mentions to also have been forced to drink from a puddle that “was probably just animal pee”, but this is offscreen.
Unsure how this is a "no" anwerr. I guess it depends how one defines "major." Three major characters (IMO) die - SPOILERS: Wilzig sacrifices himself, Moldaver is wounded in battle and dies shortly after, Lucy kills her mother after its revealed she is a ghoul. Though we don't spend much time with the characters, they are each absolutely key in the plot.
A mutated character kills a friend that is descending into being a feral (zombie-like)
***SPOILER*** a mutated character is put out of their misery in the last ep
Characters with acquired disabilities, notably an amputee and an individual with only one eye, are played by able bodied actors. No congenital disabilities are depicted in this episode.
It’s somewhat vague on whether or not Lucy’s mother left her husband to be with Moldaver or if they were simply friends. It does lean pretty heavy on the former though

Ultimately there is no attempt to rehabilitate them.
The vaults generally give off some vibes because there are many traumatized people living together and attempting to be peaceful with one another. Vault 4 episode 6.

A man tries to con an amputee with “foot healing” serums. A man with one cyclops eye is treated with disgust and a horror sting, other people with facial differences are treated the same and revealed to be cult experiment victims trying to trap our protagonists. “Crazy” and “Insane” used. Villain with facial difference trope
In-universe one episode, a Native American man discusses being typecast as a "mysterious indian" in Hollywood. But this is played as a joke on Hollywood’s expense, and he is not misrepresented by the show itself meta-wise.
I’m again saying yes on a technicality. There isn’t modern English/societal hate speech but there is speech that is hate speech within the Fallout universe regarding ghouls (irradiated humans) including barring them from establishments and calling them things like “freaks” and “abominations”
