a man may come up to the player and initiate dialogue confessing that he has been following the player around for a while if a Sunset Sarsaparilla star bottle cap is picked up by the player, he will then proceed to explain (if permitted) that these are very rare, and was surprised that the player showed no reaction to have found it
a woman, among her children, who has been captured by the legion, tells the PC that in the moment of being assaulted her husband ran away, she says that she is relieved that she and her children are at the very least not around him since he had been physically abusive with the bunch
Not directly on-screen, but there are several vaults where the inhabitants historically were gaslit (experimented on without knowing it). It is only shown through environmental clues and texts, afaik.
there are two quest centric NPCs that deal with alcohol and drug addiction. there is a drug dealer called dixon who happens to be an addict as well. you can find drug users just about anywhere but these are the most prominent. there is also cass (companion) who struggles with alcoholism
It's a video game without motion capture animations. You will probably have to fight them until you have the animal friend perk. Avoid the Old World Blues DLC (Midnight Science Fiction Picture quest)
Even if you get the animal friends perk as others have mentioned, wild animals can fight and kill each other without your intervention. There is also at least one quest about animals being killed (The Screams of Brahmin)
There is an animal fighting ring. You will likely have to defend yourself from hostile animals (feral dogs, giant geckos, genetically engineered chameleons, and large insects), and can kill crows, giant bighorn sheep, and two-headed cows
Your companion dog Rex can whimper when you dismiss him as a companion or call him to use ranged weapons.
There are a few other occasions where a dog whimpers. I think the dog of Old Lady Gibson whimpers before she gets put down in Rex' companion quest.
One quest can be resolved by paying an older woman to give you her elderly dog’s brain. The brain is uploaded into another dog’s brain, however, allowing the two dogs to live on as one animal.

and there is also a legion slave at the fort who, if your character is a woman, warns you that she overheard some of the legion officers talking about "trying you out", despite this, it does not happen
Debatably. The base game features little breathing problems but the Dead Money DLC features a toxic cloud that will continuously poison the player character the more they breathe in. However, in practice, this is more of a corrosive material then one that causes breathing problems.
A character is mentioned to be set on fire and thrown off of a cliff. This character did survive however, and can be interacted with in a DLC where he is wrapped in bandages due to the burns he sustained.
Centaurs are a horrific, tumorous mutations with misplaced limbs, massive prehensile tongues, and an awkward shuffling motion. They're mostly found in one or two easily avoided locations.
Other mutants such as necrotic ghouls, buffed up super mutants, two-headed cattle and various giant arthropods are also present, though not quite as disgusting.
There is a game mechanic where limbs can be 'crippled' during combat. It is unclear if this means a bone has been broken or if there is just a severe wound of another kind. This status effect can be healed through the use of items or rest.
live crucifixion is constantly practiced by the legion (graphic) + slavery (graphic). + gomorrah's sex workers are heavily implied to be sex slaves. a gomorrah sex worker is revealed to have been tortured to death (extremely graphic) during the quest How Little We Know
-During one of the endings, there is an option to throw a man off of a dam. -Fall damage is also a component of the game, so if you happen to fall off of a cliff or high building you will likely take damage and possibly die. -There is a character mentioned who was thrown off of a cliff.
sometimes if the PC aims for a headshot on an NPC the head may explode and the PC can sometimes see a few of the NPCs head organs scattered on the floor
Your player character's family is not shown in game. Their is an option during a psychiatric evaluation during character creation to respond to "mother" with "regret", however that could just be that you regret running away or something.
it is mentioned by Col. Hsu that the fiends are known for kidnapping civilians. there are about 4 NPCs in Vault 3 who tell the PC that they have been kidnapped by fiends. on the Mayors terminal in nipton, the kidnapping of travelers is also mentioned. the legion is often kidnapping civilians, turning them into slaves
There are "Ghost people" in the Dead Money DLC, but they are mutants rather than actual ghosts. Night Kin are semi-invisible enemies that are mistaken for "demons" by some characters. The Ghost of She is a flaming bear spirit in the Honest Hearts DLC.
There are traps such as land mines, bear traps, and shotguns that can trigger suddenly. There are also mostly-invisible enemies that appear only as they attack the player. There are frequent jumpscares in Vault 22, the Dead Money DLC, and the tunnels in the Lonesome Road DLC.
Several small unnamed lakes, Lake Mead, Lake Las Vegas, and the Colorado River can be visited in game. The main plot involves two factions fighting over Hoover Dam
It is implied in the quest How Little We Know that the PC listened to snuff tapes (murder and sex, usually used for masturbation) to gather evidence against someone. No audio is actually heard for this.
The main base of the Powder Gang is a former prison where they were held but eventually they revolted and took over the prison. Raul is imprisoned on Black Mountain by a super mutant.

There’s a quest where a schizophrenic character fights more violently than before if they sustain too much damage. This event is required to proceed to the next phase of the quest.
There is a character (Dog and God) with DID in the Dead Money DLC, though whether or not it's misrepresentation is up for debate. I don't think it is, personally.
It's more implied than anything, but a man can be found after a Legion raid with his kneecaps smashed. You can offer him Med-X, and if you give him several doses, he expresses excitement that he has enough to overdose.
Primm Slim, the robot inside the Prim casino, clears its throat when asked about the hotel. Jas Wilkins in Sloan also does it when asked about being from California.
Ghoul characters are disfigured by radiation and some will comment on discrimination or hardships directly related to their decaying bodies. The character Chris Haversam speaks about himself as if he is a ghoul but is obviously human.
To complete the questline for the Boomers faction, you need to swim to the bottom of Lake Mead to raise the B29 bomber that sank/crashed landed their. Vault 34 is partially flooded, as are a few caves
if a grenade or a similar explosive is thrown near the player the camera may shake for about a second. larger explosives like mini-nukes or exploding cars (these only explode if they are shot at) may make the camera shake with more force and for a bit longer
there is also an add on in which you must detonate warheads. this will make the camera shake violently
No, though it's implied that the NCR frowns upon gay relationships, dubbed "friendships" by Major Knight if your male character has the Confirmed Bachelor perk.
there are special dialogue options if the players intelligence is 3 points or below. if the dialogue is selected most NPCs may make offensive remarks on the players intelligence. outside of this there is the occasional "braind**d" word being used
All adult characters can die, except for companions, who only die in hardcore mode. Arcade Gannon, Veronica Santangelo, and Christine Royce are confirmed gay companions, and Cass is confirmed bisexual. There are another five confirmed gay or bi minor characters. The player can take the "Confirmed Bachelor" or "Cherchez La Femme" perks to flirt with same-gendered NPC's. Quite a few characters, including every prostitute, will have sex with the player regardless of gender.
You can shoot the powder ganger guy first if you would like. I did it on my first playthrough not because of his race but because he was threatening the woman in the saloon saying he would burn the town to the ground.
women are forbidden to serve in caesar's legion as anything but a slave (with an exception of the PC, if a woman. even then, misogynist remarks are made). its explicitly stated by many NPCs and even the legion itself that the legion considers women inferior
The sex workers at the Strip. Visitors will cheer them on and often make comments about them. Some NPCs will refer to the sex workers as "merchandise".
you are able to hire sex workers. there are also specific perks that may allow you to have sexual intercourse with NPCs. + the typical fallout sexual remarks that... many NPCs make throughout the game
The ending is determined by the player's actions and is presented as a narrated slideshow. In some ending certain characters will suffer unfortunate consequences or death.
It is possible for the player to drown, although the time limit is fairly generous. The Rebreather negates this effect, and only two minor quests involve any underwater content, both of which can be done after obtaining the Rebreather.
NPCs can be killed in bloody ways. With a certain perk, NPCs will explode into a bloody mess of dismembered body parts and fleshy chunks which are visible.
You can dismember/cut limbs.
The Legion and Fiends have some of their victims' severed heads on stakes.
