Depending on whether one NPC's quest is followed, there is a point where they offer to sacrifice their life by distracting a horde of enemies so that the player may pass through. They don't have to die if the player fights with them, but if the player chooses to run through and leave them behind that will be the end of the character.

Sort of. There aren't any scripted jumpscares, but there are definitely times when an enemy can sneak up on you and frighten you or attack from off screen. There are also enemies that hide and can grab you unawares. It's not particularly offensive, and it isn't really a horror game. There are definitely scary parts though. There is an area that is very dark and you are required to carry a light source, even when you do you can only see a short distance and enemies can and likely will appear out of darkness and scare you.

Some nonhuman enemies have spitting attacks. As well as attacks that cause clouds of gas, no farting that I know of.
There is a throwable item called "dung pie" which is poop.
There is also an opportunity to perform a critical attack on a boar where the player stabs a sword up it's bottom.

There is evidence supporting however that they are in fact a trans woman (or atleast not cis).
The game never really addresses this and the character's own opinion is never stated.
It is likely that the developers had no ill intent with the character.
Like any community though, there are people that have some very bad opinions and make bad jokes about the character. Though these can be avoided.
