There is inherent body horror in the film's premise of having your face surgically swapped with that of your worst enemy. The actual surgery scene downplays the blood and gore, depicting face removal almost like popping off a panel on a car. A guy who is getting electro therapy is shocked for so long that he v*. It doesn't show the actual act, but after a couple seconds of being electrocuted the cam cuts away and then comes back to him, and he has some v* down his shirt and around his mouth and on his beard. He escapes and we see him fighting for a few minutes with the stuff still on him. I have VERY bad emetophobia and it didn't bother me much. It's very little and you never see it happen. If you have to skip it, it's when they take Nic Cage into electro-therapy and it shows the guy before him getting electrocuted with a big thing on his head. I wouldn't recommend skipping though, as it's a cool part that is substantial to the film in full.
