Early on in the game, a major character hands the player a pack of cigarettes in a cutscene. While the player character claims he doesn't smoke, these can still be equipped in the secondary slot, with the description "Highly addictive and hazardous to your health." Whilst these are equipped, the character's health slowly drains over time, but steadies the player's aim in first person mode and reveals laser tripwires at certain locations. The player can also pick up tablets of the fictional drug "Pentazamin" which, when consumed, temporarily steadies the player's aim in first person mode with no health penalty, though this effect has to be renewed by consuming another Pentazamin every so often while aiming.

In areas of the game with large amounts of water, sea-lice are seen scattering around parts of the floor. Using a can of compressed coolant fluid, you can force the bugs to move away temporarily, but crawling on these sea-lice will result in one getting into your ration stash, and will slowly eat them away until you either run out, or the sea-lice is shaken off by rolling.

In a late portion of the game, one of the Major Antagonists Vamp, catches the character you need to escort to the computer room, EE. When he does this, Vamp will gradually choke EE out, and you have to stop Vamp before he kills her.
In the torture sequence prior to the final boss, the main villain of the game tries to choke the information out of you with his cybernetic tentacles. Failing to mash the button to hold your breath long enough and fast enough will result in a game over.

Not direct electrical torture, but at one point there is a door that has an electrified floor panel in front of it; stepping on this panel will electrocute the player character, causing them to react in pain as the camera rapidly cuts to several camera angles before he is thrown back to the floor. If the player's health is too low when they step on this panel the electric shock will kill the player character.
