In The Heist DLC, Black Cat gaslights Spider-Man into thinking her son is kidnaped and, due to their prior relationship, is also his child, a fact he struggles with throughout the entire dlc. It’s revealed at the end she does not have a child at all and manipulated him just to steal all the Maggie gang’s wealth.

I believe there is light mention of addiction/recovery in posters and flyers on the wall in the homeless shelter featured in the game, but these are extremely easy to avoid/overlook

Extremely brief imagery of dead lab rats, if I recall correctly.

There are spiders mentioned throughout due to the character in question, but if i recall correctly they are only directly shown three times: once in the opening cut-scene, once in a penthouse stealth level later on, and once in a cut-scene not long before the end of the game.

I don't recall any bugs other than spiders being shown? There are mentions of scorpions but just because one of the villains is named "the scorpion" and features a scorpion-like tail that poisons the player.

Next time I play through the game I'll take some notes on this, and update my comments where applicable.

No, but there is a sequence where multiple characters get trapped inside of a burning building. Everyone in the scene survives, though. After the scene, it is verbally confirmed that "everyone is safe". Still felt worth mentioning this for anyone who might have fear of fire/burning buildings in general

in both the main game and the dlc there are multiple hostage situations, in the dlc there is mention of a hostage situation involving an unnamed child, but this is later revealed to have been untrue. In the main game there are also two different adult men who are kidnapped, both ending up being saved by spider-man.

There are several medical scenes, including visuals of people lying in hospital beds as well as various types of medical equipment. The player's character is also mentioned to have spent a night in a hospital at one point during the game, but this is not actually shown.

Yes. One of the villains has spinal cancer, but it is an extremely minor plot point, only being mentioned about twice in the entire game.

No clue why so many are saying no. The characters who do are all either minor characters or unnamed altogether, but multiple suicide bombers are briefly shown as well as a man committing suicide by way of a gun.

In the DLC there are suggestive conversations and vague-ish mentions of sexual acts. Nothing explicit is shown or described, though.

yes, and there is quite a lot of it. very few deaths are shown in game, but most of the game's enemies wield various types of guns. There is also a scene in the DLC where a police captain directly attempts to kill someone with a gun. They are unsuccessful, but it can still be a difficult scene to watch nonetheless.
