There are multiple scenes where something magically (this is a fantasy show) leaves someone through the mouth, though not quite as traditional v.

Jessie Putnam spits fire at Sabrina somewhere in the beginning of the first season.

In chapter 12, Sabrina drinks a large amount of a potion described as "rancid", and immediately expels it twice. It's visually like the action of vomiting, but if it makes you feel better, it wasn't in her body long enough to belong to the same bodily system - technically spitting up. Sabrina makes reference to "puking" immediately after, though.

In chapter 13, a frog emerges from Zelda's throat. Not even close to resembling sickness, but be warned if you're sensitive to things coming from the mouth. In the same chapter, Salem is seeing lying next to a vomit-like puddle - it is implied that he is sick.

Spoiler because that's what you're here for and I'm annoyed I didn't know to expect it: Sabrina dies. Seriously. Both of them. Nick too, but that's played off like a happy ending because they're together. Everyone else is fine but mourning Sabrina, so YES, I count that as very sad.
