It’s possible for your party to be eaten by the Zone Eater in the World of Ruin- thankfully, it’s not graphic at all, and you can even recruit an optional character this way
While it's not an actual assault (and a thwarted one at that) Setzer's fixation on Maria and attempt to kidnap her to marry her is very creepy and gross and incel behavior. It's also heavily implied that Edgar is a shameless pervert and flirt who will even flirt with a ten year old.
Certain attacks can cause characters to enter a state called "seizure". This has no visible effect (unusually for a game where almost all status effects cause visible effects), but the character's HP does drop steadily.
Locke's girlfriend falls from a rope bridge in a flashback.
A villager is seen falling in a chasm when the Floating Continent rises.
Celeste will jump off a cliff in the World of Ruin if she allows Cid to die.
When Terra first enters her Esper form, she lets out a horrified bit-crushed scream before flying off. This scream is notable as the characters are otherwise voiceless.
Terra begins the game under the influence of a device called a Slave Crown which turns her into a mindless slave, and was made to slaughter dozens of soldiers in a flashback. She's horrified by it as soon as it's removed.
At the beginning of the World of Ruin, Celes attempts suicide by jumping off a cliff if Cid dies, but she survives. Afterwards, she finds Locke's bandana, which gives her hope again. This is (poorly) censored in the English SNES release.
Teen childbirth at that. Katrin in ruined Mobliz. There is also Madonna giving birth to Terra, though both of these births aren't graphic due to the limitations of the graphics...
No. This is worth noting because a certain infamous guidebook from around the time of the game's release claims that this game tackles abortion as a topic. It does not.
The closest thing that happens is when a pair of teenage lovers finds themselves expecting in the wake of the apocalypse, and they don't know what to do. Abortion is never mentioned, and in the ending, as the camera pans over their village, we see that the baby has been born.
While it's reasonable to extrapolate that a pregnant woman died during the mid-game cataclysm, none are shown. Disregard the weird comment about cutting babies out of people; Terra's born before Geshtal et al show up.
A barmaid teases Cyan by naming her left and right bosoms. Terra's Esper form glows through her clothing. In some versions, Siren only wears a tube top and elf shoes. Even when she's edited to fix this, she wears booty shorts that look like they were painted on.
