Not in game, but many people in the fandom ship the protagonist, Trigger (who does not have a confirmed sexuality and is only known to be male), and some female characters, especially Avril Mead (aka "Scrap Queen"). Some may headcanon Trigger as asexual/aromantic (especially if they see him as a representation of themselves), but nothing in official material directly confirms or even implies anything about his sexual and romantic orientation.
During the DLC mission "Anchorhead Raid", you have to fight 2 siblings who are hired to kill you. If you shoot down the older brother (Rage) first, his younger sister (Scream) starts freaking out, as she always followed after her brother and doesn't know what to do on her own. She starts flying erratically and struggling, and asks the player to kill her. When she is shot down, she intentionally chooses not to eject and dies.
Mihaly exhibits uncontrollable urges to indulge in risk taking behavior in the form of test flights and aerial combat despite the negative impacts on his family, health, professional career, and Strangereal's geopolitical stability.
While we know Mihaly is a predator, and have it on good authority that the weak get eaten, he didn't technically eat Brownie, despite her namesake being so tasty.
While it may or may not have been a broken bone, Avril suffered serious trauma to her leg during the game's opening cut scene and walked with a limp and brace for the remainder of the story.
Mission 17 dialogue from Osean ground units includes discovering bodies of civilians who it is heavily implied were mass murdered by an Erusians military faction. During the exchange Osean soldiers discuss wanting to at least give the children a proper burial during their retreat.
Though one city that the player fights in has two twin towers (not THE twin towers), these have little resemblance to the World Trade Center. In the first iteration of the city in the series (before 9/11 happened), however, they were exact replicas of the WTC. In another previous iteration (this one after 9/11) they were removed.
Fictional in-universe racism against Belkans (in-universe country) is mentioned, notably when a group of Belkan civilians and even children are massacred by Erusean nationalists.
A character suffers an injury that reduces their mobility and leaves them in a brace.
A character annoyed with them calls them a name once then goes on to support a completely unrelated nickname regarding they engineering ability.
In addition to armored ground vehicles like tanks being destroyed in explosions, a car chase in one mission leads to several cars crashing. This is not easily seen as you are in an aircraft (usually) high above the chase.
Yes. Every single mission of the game and can possibly include your own player vehicle if you don't continue to resist gravity as actively as you need to, but it's really easy to happen in clouds so watch the altimeter.
Not directly shown or strongly implied but entirely possible in a game where you fly a plane that is allowed to shoot missiles at big boats in the middle of water.
