The golden ending requires the game to be completed three times on a single save file; one for each active Elder God in the pantheon. Ain't nobody got time for that.
Each Ancient shows Alex their plan for World Domination when the game is completed. None of them are pleasant.

The Pillar of Flesh involves herding human sacrifices into a mould, and drowning them in concrete. Rinse and repeat until it's 10 stories tall.
Ullyoth zombies are thought to have drowned before their transformations (which is wierd because none of the areas are near bodies of water sufficiently deep to drown a fully-grown human being).
