There are multiple cases of dogs becoming hellions, but the hellions are only shown as wolfs and wolf-like creatures. In the main story there's a scene with a lot of wild dogs lying on the ground but they're breathing, the harm done to them is implied but not shown (no injuries either). However there is one case of a dog and a little girl dying together in a missable sidequest in Lastonbell.
the dungeons are filled with hallways that can feel a bit claustrophobic after a while and the buggy camera can zoom in real close while fighting which can make that feeling stronger but nothing more.
