In the beginning flashbacks some asylum workers have their clothes ripped off and chests exposed- nothing other than this is shown but the potential interpretation of sexual assault is pretty obvious. There is a moment where the audience thinks a character is dead and someone begins to touch and kiss their “corpse”, a moment later revealing they were alive and the person was their partner and they wake up okay with it but still made me uncomfortable for a moment so will mention that one as well.

There are two or three we see, with the knowledge there are more. It is later revealed in a scene where a character is tortured that the nurses were sexual sadists and would pleasure themselves while torturing the patients. We also see a naked character chained to the ceiling, framed as being post-coital. It is unclear if they are consenting. In a post credits scene, a female character who had died has replaced one of the nurses in a recreation of a part of the opening scene, implying she's now being raped by the spirits of the house as a kind of hell.
