Some of the nurses have unusual long arms. One of them has an unsettling, elongated face. One nurse has an unsettling, creepy smile on her face. The big enemy in the bathing area is bulky and has a large face and one huge brush as hand instead of a normal hand. One enemy in the final part of the game has a scowling expression and a large, round open mouth. A giant wheel or something is pinned to his back and he looks in constant pain.

Near the ending, the player has to go to a dental appointment. Once the player sits in the chair, the nurse puts on a dental head device and tortures the player by applying pressure to the jaw, causing a black screen which high lights teeth cracking visuals. This is a pixelated game, so it's not too graphic, but still disturbing.
