PaRappa, a sentient cartoon dog, can (in most versions of the game) play in a level that takes place in Hell, implying that he died to get there, though unlike the game's other protagonist, there is not a cutscene that shows him dying beforehand.

Lammy, a lamb, dies and goes to Hell in some versions of the game, though she comes back to life shortly after. It should be mentioned that Lammy is of roughly human sentience, so whether she counts as an "animal" for the purposes of this question is up for debatable.

Many important characters are cartoon animals who can be briefly saddened by various events, such as losing a level.

In the European and Japanese versions of the game, the main character dies and goes to hell. She’s revived by the end of the level, however.

Not quite, but Lammy is often shown panicking and in a hurry, depicted with anxiety.

In Lammy's panic scenes *and* when she gets the bad/awful rank in the "Vital Idol" level.

In some of the versions, Lammy will say "If I'm dead...then the game's over! What a st*pid game!"

Lammy says "st*pid" in one of the versions of the game, which is a commonly used derogatory term. The pilot ever so slightly depicts stereotypes of PTSD from war (With, when his head is hit by the control panel above, he goes from mellow to raging in a matter of seconds and makes Lammy fly the plane, assuming she's a pilot.) Otherwise, the game is clean of this.
