Multiple. Towards the beginning of the game, the train suddenly accelerates and sends its passengers flying, knocking Tails, Amy, and the player character out. Soon after, the three find Sonic laying on the ground unconscious, though he assumed to be simply acting (he isn't). Towards the end of the game, Sonic awakens and recalls how he was hit by a blowdart, knocking him out. Then, the train rapidly accelerates again and the player character falls unconscious once again.

Almost at the end of the game, the protagonist begins to break down when realizing what was really happening and how they didn’t help and act in time.
Several “choices” would then appear in the screen, all of them being thoughts of self-guilt created by the main character. Sonic quickly notices their state before it escalates further and helps them get out of the spiraling thoughts.
