During the mothers attack at the end of the episode she chains up the father with a bike lock around his neck and chains around his body. She uses a hammer on his face which does cause intensive swelling and in the next episode it’s suggested even worse damage was done to his eye. He get reconstructive surgery and his eye is okay in the next episode but bandages are used for parts of it to cover the eye. There is no enucleation.

The mother descends into madness and at the end of the episode ties up her husband and beats him with a hammer. She also boils water in a tea kettle and leaves the room to find their child and used it in some way on an infant. Because the main character is the father and he loses consciousness we don’t see the murder of the infant or hear more than baby cries thank god but it’s tense and upsetting.

At the end of the episode the mother is in a state of what seems to be possession and ties up the father and murders their infant son. The murder is not on screen but you do hear a baby crying. The way the child is murdered is not revealed, it is done in a separate room off screen but seems to involve hot water.

First in a flashback the parents of the wife die, the mom wants to kill herself and her family and she sets the house on fire. The father manages to save the children but goes back to save the mother and dies himself (seemingly at least).
In the main story the wife descends into madness. She ties up her husband and then kills their infant son Brian in another room (off screen). The husband is briefly tortured on screen.

Changelings and faerie children were the first historical references to autistic children. I have not watched the whole show however the mother is convinced her child is not her own, in photos it appears to be a different child and it cries with her but not her father. She’s so convinced that she ties up her husband and takes the baby into another room and murders it. This is something unfortunately that happens even now with children with special needs. As the parent of an autistic child and being on the spectrum myself I feel it may trigger autistic audiences.

Apollo's friend in mentioned to have PTSD both in a joking way, and later with a real reference to his issues with living in a basement.

A baby is crying on and off throughout the episode.
At the end of the episode you hear the baby crying as the mother goes to murder him (off screen). You do not hear death wails but it is upsetting.

About forty minutes into the first episode the main character and his wife are stuck on a subway in New York while she is in labor. She gives birth on the train and both she and the infant survive. There is light gore/blood on both characters as well as the baby. I had to fast forward this scene as it was several minutes of tension. It did not seem like they showed a lot of the bloody parts of childbirth but they definitely spend several minutes of active labor on screen.

There is a scene of childbirth about 40 minutes in lasting 5 minutes where the characters are trapped on a New York subway. However the mother and child survive the childbirth with the help of the father and no one dies. The scene is very tense and there is blood.

At one point Apollo tells his presumably mentally ill/postpartum wife to 'take another pill' in a cruel and dismissive way.

Very devastating. The mother descends into madness and thinks she’s being followed and that her baby is not a real baby (hence the changeling name based on stories of fae children). She tried medication to no avail, leaves home, comes back, ties up her husband. Kills their infant baby before it fades to black.
