While there certainly are a few times the dog almost dies, such as when you find "patient zero" in the biker hangout and it explodes, gravely injuring the dog, and near the end when "king" injects the dog with a syringe, stopping its heart and mere minutes later when you and the dog get hit with an airstrike. At the end of the game/when the credits roll the dog is still completely fine.
