There are allusions to a character's pet(s) or other animal companions being abandoned after said character's death. Sometimes, the pet(s) are then adopted by friends or family, but this is not always the case.

There have also been allusions to a child being subjected to human experimentation near the ending.

There is a major subplot which involves characters being inflicted with a curse which induces auditory hallucinations, which culminates into a server event where many characters are trapped in and forced to escape the dreamscape while being tormented by further auditory hallucinations induced by magic similar to the curse. While the subplot is somewhat skippable in relation to the overarching plot, it is fairly significant for individual characterizations.
There is also a major character lore stream in which the POV character can hear "music" that other characters can't; not even the viewers/audience can hear this "music".

A third character has sibling death as part of their traumatic character backstory, and the circumstances surrounding that sibling's death is extensively discussed in the finale and drives that character's entire storyline.

One character is repeatedly described to be homeless as visual symbolism for them never feeling at home in a toxic familial environment and having lost their "true" home before the series began.

The series is set in the middle of the ocean. What else do you expect? /lh

While multiple queer characters die over the course of the SMP, none of them (as far as we know) have died due to their GRSM identity.

It's briefly mentioned that a married couple both cheated on each other with the same character (the POV character of the sub-plot, who is pan), but this is skippable and doesn't play a major role in the plot at all.

A few characters are repeatedly referred to with pronouns they do not use, but this is entirely unintentional.

There are a couple of extensive lore scenes where a character (or multiple characters) are held in captivity in a large cavern after being kidnapped, among others.
One of the crypt puzzles in the server mechanism also involves having to go through a maze of doors and corridors to escape; this is entirely skippable.

At the end of the series a strange sea-urchin creature named Cruppy first introduced on the first day sacrifices itself to stop an eldritch force.

A couple of baby Komodo dragons (monitor lizards) die on screen, but I don't think this is what you're looking for under this content warning tab.

There is a flashback animatic in 2 POVs of the finale in which a character is audibly stabbed in the eye, complete with graphic sound effects.

No cars appear in the setting outside of a brief and skippable cinematic filmed in live-action, and no sound effects of the sort are present in said cinematic.

Other than in-game drowning, there is a climatic scene near the ending where a character shoots out the ice under them, falls into icy water, and nearly drowns; they are rescued but rendered unconscious for over a week of in-universe time afterward, and eventually wakes up coughing.

The plot twist of the series hinges on the setting being founded on a lie.
There is also an unintentional sequence in Chapter 1 where something disturbing is seen by only one character, whose companions don't believe them and are subsequently convinced they were seeing things (gaslighting oneself?) until a different character affirms them by having seen the disturbing thing themself on a different occasion.

It's only as graphic as it can be in Minecraft (which is to say, not at all), but there is a semi-comedic lore stream where a character decides to start a "revolution" to free the cows from a crowded pen in the name of "cow rights", only for most of the released cows to die anyway… I guess.
