The ending in question is one of three possible actual endings and is relatively easy to avoid. However, the many death scenes might also count as bad endings.

Other than the violent deaths from the game overs and the zombies, a fridge full of bloody raw meat is depicted and eaten, though this is treated as normal as the characters can safely eat such diet as it is.

Some of the sound effects are wet, especially the ones with gore. I know there's a trigger for that specifically but it's possible someone might not consider that, especially if they're just checking for someone else.

One bad ending has a character having all their bones broken. In the route of the good ending another character "accidentally" (didn't want to, but I am trying to not giving spoilers) breaks the leg and arm of two other characters, who sustain these injuries in the sequel.

There is a line near the end of the game that suggests that Patches dislikes people who love everyone, which refers most obviously to Olive having at least mild crushes on everyone around them but could be interpreted as him disliking pan and omni people, or those into polyamorous relationships.

A major character switched bodies in the previous game, and there's a lot of mentions of how it negatively impacts them in a way that could be similar to usual body dysmorphia.

Unlike the third game, attention isn't drawn to it, but it wouldn't make much sense for Patches to *randomly* find Olive and Sparky in a bunker in the middle of Kemono Woods. Plus, he already has a track record for it from the first game.

Character is restrained and spits at who restrained them. This is in a bad ending and easy to miss even if you do this bad route.

The game has an equivalent of hell. While it being shown in the other two games, it isn't seen in this one (if you don't count limbo as being hell). One character can be sent there though, but the route isn't the canonical one.

One game over has a character unable to move, it being implied before a fade to black that the antagonist will "test their new ability" out on the paralyzed character. Another bad ending involves a character being found dissected and yet still alive.

There's a bad ending where this happens. However, said ending isn't part of the achievements, so you can 100% the game without going through it (make sure, when the option is first presented, that a certain character doesn't go through the gates into the woods without you).
