We see a lot of dead fish with one long scene in particular which involves some unpleasant images and dialogue about putting fish on hooks. Then they come back from fishing with some big dead fish.

There is a pretty graphic and long strangulation scene between a male and female character. The scene shows both of their faces. The female's face is in distress, and the male is agressive and angry. You see it from both the victims eyes and the agressor's eyes as well, which was personally very triggering. It is skipable, but it is a pretty long scene.

This film is pretty groundbreaking in that none of the patients are violent or dangerous except for McMurphy, who is not actually mentally ill. They’re all good people and it’s the staff who are violent. The one secede of violence by McMurphy is completely justified

Sad but also hopeful. (Don’t read on if you don’t want spoilers) McMurphy is left brain damaged by ECT and the Chief smothers him with a pillow as a mercy killing. The Chief then takes back his power and autonomy by picking up the control panel in the hydrotherapy room (which had previously been shown as being too heavy to lift) and smashes a window with it, echoing one of McMurphy’s earlier escape attempts. As he runs away from the institution, the other patients cheer him on.
