There are big, praying mantis-like creatures, that turn out to be faries in diguise. Also there are lot of big bugs in the scene with the toad. They crawl on the protagonist's arms and face. Though they look kind of cartoonish, so they are not that triggering (I have a bug phobia but these were relatively okay).

There's a creature with his eyeballs not where eyes usually are: they are on a tray on a table and he can attach them to his hands. So eyeballs are shown there, and the creature is scary. Also, the stutterer guy's eye is swollen when they show him after having been tortured.

SPOILER---------------------------------------------------------The protagonist is shot at the end. It is quite open to interpretation if she's reincarnated or the last scene is just imagination. But she is clearly shown dying once. Also there's attempted harm towards a newborn baby, but nothing happens to him at the end.
