[SPOILERS] It's worth mentioning that the scene near the end where Homura grabs Madoka by the hands, splits her from her goddess form, and says "Now... I finally have you" is considered by some to be analogous to/a metaphor for sexual assault, however, this is dependent on each viewer's interpretation.

No kids die, but we do see young teenagers making suicidal gestures or in situations where they come close to dying.

- Homura (a 14-year-old) shoots herself in the head to catch Mami offguard and it leaves blood on her face, but she doesn't die since magical girls can only be killed if their soul gem is shattered or otherwise irreparably damaged. Shortly after, she freezes time and points her gun at Mami's (a 15-year-old) soul gem, but decides not to shatter it and shoots at her thigh instead.

- During the battle with Homulilly, Sayaka (same age as Homura) drives a sword through her chest and blood comes gushing out - she's unaffected for the same reason.

- A bit later into the same battle, Sayaka and Kyoko discuss the former's death in the previous movie, Eternal (or in Episode 12 of the show). By the movie's end, she's reformed back as a normal living magical girl against her own will.

- Homura, after Homulilly's defeat, is seen laying on her deathbed and is about to be taken to heaven by Madoka. She defies her death, however, and usurps Madoka's powers then rewrites the universe to where everybody is alive.

[SPOILERS] It's complicated; Homura is heavily implied to struggle with PTSD, depression, and suicidal thoughts, as well as suffering what appears to be a psychotic episode during the course of the movie. She's not outright violent, per se, but she's shown multiple times to resort to violence when she thinks there's no other options (as well as willingly putting innocent bystanders like Bebe in mortal danger because of a misunderstanding), and the ending where she usurps Madoka's status as a goddess further confounds this.

No main character directly commits suicide, but there's several scenes where characters appear to be committing suicide or make suicidal gestures (see my answers under self-harm and kids dying for more thorough explanations).

Near the end of the movie, Homura's familiars, the Clara Dolls, are shown to be jumping to their death - this is made more obvious by them removing their shoes before jumping, which, in Japan (where this movie was made), is a ritual done before someone commits suicide.

There is a post-credits scene where Homura jumps off a cliff. It isn't clear whether or not this is a suicide attempt (as Homura can't be killed by normal means), but it's safe to infer such is the case as there's lots of subtext about Homura wanting to die.

It's arguable whether or not this is a happy ending.

Madoka's powers are usurped by Homura, who also becomes a god (or a demon, as she calls herself) and rewrites reality again. In this new reality, nobody dies and everyone lives happily as normal humans, but this was done completely against their will. Sayaka resents Homura, makes it clear she's out for vengeance, and has to relive the pain of being rejected by Kyousuke.

Madoka, even though she was stripped of her powers and memories, instinctively knows that she's destined for a greater purpose. When she tries to reconnect to this, Homura suppresses her attempt.

There's some implication Homura is unhappy with taking away Madoka's powers, however, this is also arguable and easy to miss.

copied from IMDB parent's guide with some minor changes- (no real spoilers, don't worry, read it all)
A character (in a metaphorical transformation sequence, as in it technically doesn't *actually* happen) stabs her chest and her (cartoon <3 shaped) heart is briefly seen on the end of her sword amidst the blood-spray if one pays attention. This is a very very short shot which is shown as an artsy silhouette, and is very downplayed and not at all intense. (genuinely- this didn't upset me at all)
A character points a gun to their head mid-fight (shown in the trailers) and shoots themself to trick their rival. Due to the nature and mythos of the story, they do not die, but it is still just as graphic as it would have been had they actually died. The next few appearances of this character on screen show them with blood lightly splattered across their face.
** about the one above: i always skip through the part where this happens- the setup is super obvious and skipping it is super easy but from what i have glimpsed, the bullet simply goes through their head in a graphic-ish but non gorey way, and blood goes everywhere. every frame after it happens is non gorey and non realistic, like literally theres just some red splat on their face that doesnt even look like liquid or blood
