Honestly, given the setting, a huge chunk of the cast aren't particularly healthy as people. Nathan in particular needs to violent by necessity, as it's part of his job ("Thankless Job" is a jaunty number about how, well, it's a thankless job, even if he has fun during the song). However, guilt is a major part of his character, and he does messed-up things outside of being the Repo Man. There is also Luigi Largo, whose entire schtick is snapping and stabbing people in a rage under little provocation in a darkly humorous way. Unlike the other siblings who seem to just be depraved, there's a possible reading that he has an underlying undiagnosed/untreated illness.
