Tamara dies after attempting to run away from her abusive boyfriend, who threatens her with a pistol. No onscreen contact is made, though he does verbally threaten her and demand she gets out of her car.
At the start of "A Penny For A Tale," just after the "Beautiful Stranger" sequence, Ms. Merryweather is forced into a kneeling position and her arms tied out from her body so that her back can be whipped.
Tamara is attached to a knife-thrower's wheel by her wrists and ankles.
Merrywood is stripped of all but her underpants--while no explicit touching or groping happens, she still protests the entire time her clothes are being taken away and then resists being dragged to a secondary location.
At the end of "The Scorpion and The Frog," Scorpion intentionally throws one of his knives dead center into Tamara's torso while she's attached to the knife-thrower's wheel. This kills her near-instantly.
One of the character's storylines is kicked off by the mentioned death of his son--it isn't seen onscreen and the kid's "spirit" can be seen throughout without injuries.
Each of the three main characters--Tamara, Ms. Merryweather, and John--die within the first ten minutes. Their deaths aren't shown onscreen, but it's heavily implied since they end up in Hell to kickstart the plot.
John is heavily implied to have slit his wrists, though the action itself isn't shown--we see John's memory of the bathroom where he died, and the mirror is covered in blood, but we don't see a body.
