The regular edition is fairly violent, considering the entire movie is based around a war and lots of injury and death is shown. I would say it only gets EXCESSIVE in the extended edition, which is rated R for violence. I wouldn't describe it as horrifically gory, but there are a lot more bloody deaths and violent moments.

There are some prolonged death scenes, but I wouldn't describe them as torturous so much as drawn out for the drama. The closest thing I can think of is a sequence from the extended edition, where a character is kept in a spiky cage in Dol Guldur and thrown around by an orc. The orc holds their hand down and raises a blade like it's going to cut their fingers off to get a ring, but is interrupted before anything can happen.
