In the intro theme (at least as of episode 2, I don’t think it was in the pilot) when the scene pans past Juniper Hill there’s a still of a (animated/drawn) kid getting a lobotomy. I don’t think there’s any real mutilation shown but it looks like the needle is touching his eye.
In S1E3 when the kid goes into the freakshow attraction and an old frail man in the dark beckons him to come closer, he lights a match revealing his extremely gaunt face and he’s missing an eye (no blood or gore, it’s just an empty socket). 3:36 minutes in. (I’m writing these in real time as I watch it so sorry this is funky formatting!) The guy comes back about 7 or so minutes in and chases the boy through the forest, and someone uses a slingshot to shoot his other eye. It’s grody but definitely not the worst eye gore, it’s pretty fake looking.
