There's head related gore in several scenes, the head is not smashed however. More than one person loses their face(ie ripped off.) one person had their face ripped off and continues to live, you see them a few times. There's also a scene where the skin of someone's face is shown, it's kind of like a rubbery Halloween mask. Someone is also shot in the eye. There are also multiple gun shots to people's heads.

A character loses both their eyes over the course of this movie. Two scenes, one for each eye. In one the person's eye is stabbed and then pulled out of the socket when the knife is withdrawn. The other is the same character getting his other eye shot out.

During all werewolf transformation scenes the transformation is shown in great detail with the premise that the werewolf has to "break out" of Lou's body. His skin splits open completely, he pees blood, his feet grow and partially burst out of his shoes. A character holds up a face-shaped piece of flesh which is revealed to have come off of the main character. This is the most graphic part of the movie but there are also scenes where the werewolf tears apart several men and the bathroom is shown in several scenes to be covered in blood, and a scene where a man has a pentagram carved into his chest is also shown.
