Yuki Takeya, Kurumi Ebisuzawa, Yuri Wakasa, and Miki Naoki decide to stay over at school. Along with the school adviser Megumi Sakura, they suddenly find themselves to be the final survivors of a zombie attack, and continue to live and survive at the school.

The teacher’s head gets squashed by kurumi. They only show a shot of the top of her head right as the shovel is about to hit, and then a faraway shot of the teacher on the ground with her head seemingly gone/squashed bleeding out.
It's implied that the main characters (aged 17-19 after spending around a year in the school) all lost their families to the zombie outbreak, as no one came looking for them; later in the manga past where the anime adapted, one character returns to her home to confirm it is empty.
While there are no real ghosts, the main character (and, later in the source material, another character) regularly hallucinates that people who died or became zombies are alive and well.
Manga only; Not an eating disorder but adding since it is related; characters try rationing food and a character mentions trying to "diet" in order to save food, someone joins in but the idea is soon abandoned due to health concerns.
The premise could be considered ableist, though I'm not sure. The main character has very strong delusions/hallunications, though they're portrayed very neutrally. They may also have an intellectual or neurodevelopmental disability, considering their "childlike" behavior and the fact that they have trouble with booksmart sorta things (though that would probably fall under the realm of headcanon or personal interpretation). Even so, the character is a good person and good friend, so I don't really feel that they're the worst possible depiction of mental illness/disability. I'd still say to proceed with caution if you're sensitive to certain things.
As with many Manga and anime involving high school girls, there are suggestive scenes, mostly just having the girls show more skin, or having them in sexy poses.
In the manga a streetlight is hit by a camper van, the aftermath is shown, but it is a minor accident where no one is injured and the car remains mostly intact.
A gun is found, but gets tossed off a rooftop.
Manga only; Soldiers, zombies, and people in office attire are shot and killed by soldiers. A major character is nearly shot by soldiers.
Not a gun, but a crossbow is used to fire a warning shot, no one is injured.
