Henry, a newly resurrected cyborg who must save his wife/creator from the clutches of a psychotic tyrant with telekinetic powers, AKAN, and his army of mercenaries. Fighting alongside Henry is Jimmy, who is Henry's only hope to make it through the day. Hardcore takes place over the course of one day, in Moscow, Russia.

The main character has prosthetic limbs added to his body in the opening, there are some shots of bodies with missing limbs, and a henchman gets his hand cut off in one action scene.

It isn't transgender body dysmorphia, but the main character Jimmy is revealed to be remotely controlling clones of himself because he was paralyzed, and he dislikes his paralyzed body. He is extremely insecure due to the fact he cannot move well and has a non-functioning penis.
A major character dies and though he denies being gay, he could be seen as gay coded and has a very strong bond with Henry. Also, the main villain kisses Henry.
Not sure why this is marked with no. There's a scene that takes place in a strip club, some of the characters are completely nude while others are barely clothed.
There is a scene where the main antagonist insults a man for having sex with a woman who he says is his wife, and he is shown to be sobbing loudly before the villain kills him.
Possibly more than I've ever seen in any other movie. Literally non-stop shooting for long periods of time. (IMDb says the body count is at least 211, and most of them are shot dead.)
