Episode 2 around half of the episode okaru-kun v*mits blood. I dont remember the exact time but around 15:30 or so. Right after he gets hit by a sumo spirit.

Episode 6 right at the end the lady in red throw up. No visuals, just audio. It is after seeing blue fire and a character explains what happened.
Episode 7 around 15m30, the mother will spit blood. Visual and sound but it's more like coughing.
