While it may not count as abuse, Madoka’s mother (at least in the episodic anime) strikes Madoka across the face when she insists on going out to find Homura during what is interpreted by the uninvolved population as a major typhoon.

Kyubey, the animal-like villain of the movie (he's essentially the "mascot critter" in appearance) is shot several times.

- Sayaka, a 14-year-old, becomes a witch and is killed by Kyoko. Kyoko is also (presumably) 14 and dies in the same explosion she kills Sayaka's witch with.

- When we are shown Homura's past (she has time-traveling powers and has relived the same month several times), we see all the main characters (the oldest one is 15) except for Homura die multiple times.

- When Madoka rewrites the universe, she takes magical girls - most of whom are young girls - to heaven shortly after they die. We see quite a few of these girls die, but they die in peace.

- While we don't see any deaths besides the aforementioned on-screen, it can be inferred that younger children (such as Tatsuya) died in previous timelines when Walpurgisnacht hit Mitakihara, as Walpurgisnacht can kill hundreds of people just by appearing.

Several- during the time that we see the previous timelines, we see at least one where Mami has a panic attack that leads to her killing one magical girl and attempting to kill others. When she is killed in self-defense, it causes the killer to have her own anxiety attack.

Additionally, there are several points that Homura has what could be considered anxiety attacks.

[SPOILERS] Kyoko kills herself in an explosion while also killing a witch. Mami is also shown on the verge of suicide in a flashback to a previous timeline, but is killed before she can do anymore damage.

[SPOILERS]

It has a bittersweet ending, though with more emphasis on sweet as there's an overall hopeful tone. Madoka's wish is to prevent all magical girls from becoming witches, thereby stopping all the girls from falling to despair and taking them to a magical girl heaven of sorts. To do this, she has to ascend to godhood, thereby causing virtually all of her loved ones to forget she ever existed. The only living characters who remember her are Homura and Madoka's little brother Tatsuya.

Sayaka dies again after Madoka's ascension, but Madoka allows her to see Kyousuke perform one more time, to let Sayaka come to terms with her death and the outcome of her wish. Sayaka ultimately ends up working with Madoka, as shown in Rebellion.

It's worth mentioning that this movie is not the finale and there is a continuation in Rebellion, but details on that will go on Rebellion's page.

No nuclear explosions, but there are a few scenes of Homura using bombs and the like, which has a similar appearance.

Alchemy
Homura lacks a magical weapon, and as such relies on mundane weaponry, including multiple types of guns- including automatic weapons, rifles, and ‘basic’ pistols- as well as explosives as mentioned above.

There are, resultingly, scenes of Homura using these guns on both Kyubey- an animal like being- and humanoids, including other Magical Girls in self defense.
