The first one happens when Slade fights Billy. He stabs him in the eye with his machete. Only the sound is heard but there's no visual gore because Billy's wearing a ballistic mask. Towards the end of the season, in the penultimate flashback (I think), Oliver fights Slade and has an option to either cure his Mirakuru or kill him. Slade threatens to kill his family, so Oliver snaps and stabs Slade in the eye with an arrow. There's an up close shot of Slade going limping his head dropping back down. The arrow is visible coming out of slades eye but there's no closeup on the mutilated eye itself

Many many times. Do keep in mind, however, that this is a show designed for teenagers. It's nowhere near as gory as shows like the Boys, Game of Thrones or Walking Dead. The torture scenes are still dark but there's no graphic ones. Probably the worst one is when Tobias Church kidnaps Renee Ramirez in the first few episodes of Season 5. He hangs Renee with chains to act as a punching bag while he beats him with brass knuckles. Also, much later in the season, Oliver is kidnapped by Prometheus and held for multiple days. He is waterboarded for a prolonged amount of time, shot with 3 arrows in quick succession and is forced to watch one of his "allies" die when Prometheus "breaks her neck" in front of him.

Early im aeason 1, Laurel Lance is kidnapped by a serial killer named the Dollmaker who forces young women to drink liquid silicone through a tube which solidifies inside their body and suffocated them. The episode is actually quite disturbing, and if you skip it, you won't miss much in the long term plot (other than some bonding between Oliver and Felicity). The big bad of season 4 uses telekinesis to choke his victims in a way identical to Darth Vader. It's best to assume that every fight scene against Damien will result in someone being choked but eventually escaping. Except one, but no spoilers.
