The worst thing that actually happens to Dorothy is that she is strapped to a table. However the medical treatment which they appear to have in mind would definitely be child abuse if they actually did it.

The so-called medical treatment which Dorothy is supposed to be getting would probably qualify as torture if it ever actually happened. It doesn't. I'm not sure why people have answered yes to this, there is no actual torture, the worst thing that happens to anyone is being hit by a lunch pail.

Mombi can change her heads, we see headless stone bodies, she wanders around headless in one scene. Jack loses his head briefly. The gump is just a head having been previously shot, decapitated and stuffed, and the head repeatedly falls off the furniture they attach it to.
