The lyrics allude to the floor show performers being drugged, as the lyrics are from the musical (in which they were drugged.) And they act strangely during the show

As mentioned above, Frank goes into Brad/Janet’s rooms disguised as the other and begins to initiate sex with them. However, before anything happens, they each realise it’s Frank and consent to having sex with him anyway.

Brad is made to believe that he is having sex with his wife. It is played for laughs when he realizes he's been hooking up with Frank

For those who haven't seen it - brad & Janet are lured into a strange house & held captive there. The show is essentially a slow mental breakdown of the two into sexual depravity. - a character is killed after singing a song, the group have dinner in a different scene and the corpse is found to be in the table, implied that they have been eating him - when Rocky disappears, the Dr. Whips his servant, but it's clear that it's intended to be played like a BDSM type of situation - when they kill the Dr, it's very slow. It's all pretty silly. 70s sci-fi violence.

Frank breaks his neck at the end, after being zapped and falling from the curtain. No triggering visuals (just his head tilting back), and the audio also sounds very fake

At the end of the movie when Rocky climbs up a light tower holding Frank, the tower breaks causing him to fall into the pool below, killing him.

In the Floor Show scene toward the end of the movie, four of the characters appear wearing makeup that isn't really clown-like but it's kind of similar? Their faces are painted white but the makeup around their eyes and lips is more realistic than a clown's. However, if you think you would find this disturbing, turn the movie off when The Criminologist (narrator guy) says "no picnic."

Not mannequins per se, but characters are at one point turned into marble statues, in the end they’re turned back (however there are similar statues around the castle and their fate is unknown)

A character is murdered offscreen with a pickaxe. The sound of the pickaxe repeatedly hitting the victim (along with the sound of the a screaming witness) is pretty gross.

Somewhat yes, a straight married couple “cheat” on each other with (+ sexually assaulted/coerced by) the same person, but it’s not biphobic about it

Rocky is shown to feel instinctively scared of people after he's physically abused. There's not enough time for him to develop a chronic condition, but he appears traumatized.

It is shown that Rocky is mummified and in a human sized tank of water. But it is not exclusively a claustrophobic scene.

When Dr. Frank-n-Furter and his assistants are about to bring his creation to life from the tank full of water, the set lights flash on and off. I don't know why so many people said 'no'.

Dr. Frank-n-Furtur uses the word “transvestite” which was the common term at the time of filming, but is now outdated. It could make transgender audiences uncomfortable.

There's a song titled "Sweet Transvestite"; however, the character is using the phrase to describe himself in a positive light. The show was written in a time when that was a commonly used phrase and as a trans person I always saw it as a reclamation type deal, however if you think this would be upsetting, I'd probably skip it.
