In the first season, a brother-sister thief duo discusses their abusive father. The sister goes into detail by saying their father hit them to "teach them lessons". She exposes a large scar on her shoulder and says her dad hit her with a bottle when she was young (<10 y/o). Says she was mostly raised by her brother. At issue in the episode is that the father puts a bomb in the sister's neck to manipulate the brother and threatens to kill the sister if he doesn't do everything he says. The bomb is removed and the sister is saved.

S1E12 Caitlin does it after she and Barry go to the bar. There’s a decent warning— “Barry, I don’t feel good.” This is the only instance in this episode. There is audio and visual, but it doesn’t last too long.
S4E4 This is the first appearance of Ralph Dibny, The Elongated Man and a bus meta. His powers manifest, and he is taken to STAR Labs for Caitlin to restore him. When he is in the bed and everyone is in the room with him, Ralph sneezes, and his face stretches off. Joe is then heard and seen getting sick. You don’t actually see it because he does it behind Barry’s back.
S4E5 Barry is heard and seen getting sick in the jail cell toilet. Again, you don’t actually see it, but you can see him with his head in the position of the toilet, and he wipes his face. Fair warning. You should have the time to skip.
Finally, each time Barry runs Diggle somewhere, Dig gets sick. Should give enough time to skip. Some are more graphic than others.

Yeah, but it isn't because they're LGBT. [SPOILER] is shown flirting with a person of the same gender once. The show doesn't make too big a deal of it. Later, the person dies due to timeline complications.
Also, one character who is openly gay does die, I think.
LGBT oppression in camps is shown in a crossover episode, but the episode can be skipped.

The main villain of the show fakes being paralyzed in order to gain sympathy. He is revealed to be the villain because he stands up from his wheelchair, which perpetuates the all wheelchair users are paralyzed and anyone that can stand up is faking myth.

A character also invents a fictional mental illness called "Lightning Psychosis" in order to explain the person in question;s quite creepy behavior towards the girl that he likes, instead of saying: "Hey he's in love with you."

A girl with DID or Dissociative Identity Disorder is shown and one of her alters is seen as a monster that attacks people, stigmatizing those with DID. It's never directly said on screen that it is DID, but with context clues you can figure it out very easily.

A partially blind character is introduced with horrible burns on his face, and is often referred to as "pizza face".
The character is eventually killed because another character uses his partial blindness as an advantage.

The main character comes back communicating differently, and is eventually revealed to have Schizophasia. The character's significant other starts crying and saying that she wants the old him back, and that she would be sad if this was all that was left of him.
