Debatable, up for interpretation. It's either a ghost or a hallucination of a deceased person. It's a woman in a wedding dress who stands by the wall in a bedroom for a few minutes, silent and unmoving, looking at the sick person in the bed. I'm very scared of ghosts, but for some reason this one didn't bother me.

I found the relationship between the Reynolds and his sister to be emotionally incestuous. I is not explicit. Nor is it per se romantic. It is definitely not sexual. But it made me very uncomfortable. She keeps his girlfriends in check and dumps them for him. She always shows up on what the girlfriend assumes to be dates, without a warning.

No. The only thing that comes anywhere near anything sexual is a scene in which the shape of a woman's b*****s (including her nipples) are pretty clearly visible through a petticoat while she's being measured for a dress. The back of the dressmaker's hand grazes the side of her breast very briefly at one point, but it doesn't seem erotically charged to me.

The ending is "happy" in a very dark way. [Vague spoiler] The main characters are satisfied despite a situation that would be disturbing to outsiders. I didn't feel as though it was a depressing film, but it will certainly disturb many viewers.
