Not finished with the series yet and it's not shown on screen but there is a scene where a prisoner talks about "docking" his dog when he was younger in pretty graphic detail and he goes as far as killing the dog. He says he's joking after, but it's unclear what the truth is.

In season 1 a character gets glass in his eye and has surgery to restore vision; not much graphic here. In season 2 episode 8; a COs eyes are forcibly removed, act is not shown but it is very graphic, showing the eyesocket. Look away from about 23:40 to 24:30

If electro-therapy is a trigger for you, you should know that a mentally disabled character undergoes ECT & is later executed using the electric chair. The chair is explained to him as one last special ECT session. The culmination of his storyline is extremely upsetting.
