In episode one Chika saves Riko from what she believes is an attempt to drown herself.
Some have interpreted the scene at the end of episode seven to be a suicide attempt, she is however stopped by her friends and never mentioned again.

Though in Episode 1, Yoshiko falls from a tree, in Episode 5, Riko jumps between the windows of two buildings out of fear of Chika's dog, and in Episode 9, You almost falls from a balcony after being distracted by a falling idol uniform (Chika and Riko save her). In all of these instances, the characters end up safe and unharmed.

Despite a meme in the fan community implying that Chika cheated on You to be with Riko (in reality, You had an unrequited crush she hadn't voiced) or that Riko cheated on Chika with Yoshiko in season two (due to the Chika/Riko pairing having far less screentime and development than in season one), no canon infidelity occurs.

In at least S1,EP7, there's a scene you can argue is self harm. Chika repeatedly hits her head with her hands while crying bc she feels like a failure. however the rest of aqours at time tries to comfort her and calm her down

In the mobile game, Hanamaru constantly complains that she's fat, even though she's drawn just as skinny as the other girls.

Some animated performance sequences have lights that flash somewhat quickly. In videos of live concerts by the voice actresses, many flashing lights are used, especially for performances by subunit Guilty Kiss.
