Jigsaw has disappeared. Along with his new apprentice Amanda, the puppet-master behind the cruel, intricate games that have terrified a community and baffled police has once again eluded capture and vanished. While city detectives scrambles to locate him, Doctor Lynn Denlon and Jeff Reinhart are unaware that they are about to become the latest pawns on his vicious chessboard.

The torture the character has is probably the less bad/focused on in comparison to all the trap victims Jeff has to save. I wont consider it brutually especially for a movie with torture in it

The first scene is a man breaking the bones in his foot and snapping his own ankle to free himself from shackles.
The first trap is a man with metal rings through his body, including both Achilles tendons. Which he has to rip the rings through to escape.
The Rack trap shows limbs being twisted, including ankles. So many compound fractures.
At the end of the movie, you see a flashback to the very first scene - the man is shown walking on his snapped ankle. It also gets kicked.

Amanda young, one of John’s apprentices, is VERY heavily coded to have borderline personality disorder. After she was traumatized by being put through a trap, John essentially manipulated her into becoming dependent on him, making her his apprentice. In this movie, she attacks multiple people and even eventually kills someone while breaking down, because she’s afraid John is going to abandon her. Can be very triggering to see Amanda as a villain if you have bpd.
