The animation for a ReDead attack can be quite distressing, as they suddenly latch onto the player character's back and hold him in place. It looks unfortunate and is ultimately innocent - as far as an enemy attack goes - but can still be triggering.
link can catch on fire in game and burn to death (if he has low health), also if you stay in death mountain crater (and some areas of the fire temple) without the goron tunic for too long (there's a timer that counts down) link will catch on fire and die

The Shadow Temple was used as an interrogation/torture chamber. The boss of the area, Bongo Bongo, is implied to have been a victim of such torture (He's literally a torso with no head and disembodied hands, if you're wondering).
when you go to goron city as adult link, the only remaining goron tells you that the rest of the gorons were kidnapped and put in the fire temple, part of completing said temple is rescuing them from jail cells
Not necessarily, but when hit in combat, Link grunts and occasionally yells. Furthermore, if Link dies he will gasp for air (drowning) and possibly yell when struck or when attacking.
If you play the original N64 version, there is blood on an enemy known as Dead Hand, which you fight in a well and then again in the shadow temple. In the first release of the N64 version, the final boss coughs up blood, although it's not very graphic or disturbing. It's green in the later releases.
