When Booker and Elizabeth enter the Graveyard Shift (bar) in Shantytown a man walks over to a barrel and vomits into it. You can prevent this by either killing him before he gets to the barrel (he walks slowly), although then you'll have to kill everyone else inside the bar as well, as they will attack you. Or you can walk in and immediatly run down to the basement, that way you'll miss the vomiting.

The game is perhaps the most graphic entry in the series. The most gruesome scene happens right before the player is able to do combat. Basically, a man's head is thrown into a rotating hook, with gory results. An innocent man's skin is also eaten by crows. Otherwise the player can perform bloody, but less detailed executions involving decapitations and slashing with the skyhook. Guns also produce bloody results including head explosions.
