On at least 3 occasions (MAJOR SPOILERS):
- Early on in the game, the protagonist and his cousin, Roman, are kidnapped by a member of a Russian mafia family as retaliation for killing one of their members and are both tied up to a chair in a basement. They are both released to work for said family, but only after Roman is shot in the stomach.
- Around halfway through the game, Roman is kidnapped by the main antagonist's gang and the protagonist is sent a message by the main antagonist with a picture of Roman tied up to a chair.
- Near the end of the game, the protagonist kidnaps a girl with the intention to hold her for ransom. She is later seen tied up and gagged before being released in exchange for some diamonds.

- If the player chooses to trust the main antagonist and go through with a lucrative deal, they are betrayed again and Roman, the protagonist's cousin, is killed at his own wedding. During the end credits, Roman's wife, now a widow, reveals that she was pregnant and her child will be born without a father (Niko promises to take care of the child). In a later phone call, the protagonist's girlfriend, disappointed in him sacrificing his morals for money, breaks up with him.
- If the player refuses the deal, they get their revenge and kill the main antagonist. At Roman's wedding, the protagonist's boss, who prepared the deal, appears furious and fires into the crowd attempting to kill the main character, but failing and killing the protagonist's girlfriend. Roman survives and he promises to name his first daughter after her.
The second ending is generally considered better, but is still bittersweet at best.
