Before going on a hunt, the player can eat a meal that gives them a boost. There are some eating noises, but they're pretty quiet and you can skip the cutscene.
It's subtle, but Shara Ishvalda and the Fatalis weapons have eyes that will stare at the player constantly – not the character, the player. That's pretty much the only fourth wall break
You can be defeated by the fire breath of some monsters, but you are seen fainting, are are either able to quickly respawn, or are returned to the main Hub if you fail the quest. So technically yes and no.
Pukei-Pukei spits poison at the player and his later Iceborne Coral variant projects a stream of hot water from its mouth as an attack. Once the player fights each of them once it is possible to avoid fighting them again.
No in general, but there can be things that are mildly sexualised. There are skins for the handler which are mildly sexualised. There are also a bunch of armour sets which can be triggering.
There is no visible blood, nor bloodstains, but body parts of the monsters the player hunts can be damaged, and in most of the monsters' cases, their tails can be cut off.
There are projectile weapons, some of which look/sound more like guns than others. It's possible to simply not use them but not totally possible to avoid them if playing online as you can't always control what other players will use.
