E14
In S10E14 (grand finale), Asia O'Hara attempts to perform an extraordinary trick by releasing butterflies from her bracelet in one of the final lipsyncs, convinced they would fly away majestically. However, the butterflies end up falling on the ground. It's unspecified if they died, as they are shown moving in one clip.

During the season 10 finale a queen tries to do a reveal with live butterflies, however it shows a number of the butterflies have died or die during the lipsync. I find it impossible to watch.

Occasionally, some teeth are covered with black ink to create an effect (usually for comedy). One example was in the Madonna Rusical S12E7, when each queen drew a gap in their teeth to resemble Madonna.

I don't think it was this kind of injury, but some queens have injured their knees or ankles (nothing bloody or too descriptive). Two examples are Eureka (during cheerleading challenge S9E2) and Kornbread (during acting challenge S14E4)

Yes. On top of that, queens are sexualised while dressed as minors. For example the S5 E11 "Sweet Sixteen" runway, where it is literally described as "a sexy sweet sixteen". Describing the looks as "promiscuous" and saying that 16 year olds these days "wanna look like a whore".

S7 EP12 the r-slur is used uncensored. skip past Kennedy Davenport's meal with RuPaul to avoid.
S7 EP4 Katya says "f*ckt*rds" (variation of the r-slur
S13 EP5 the lip sync song is "Fancy" by Iggy Azelea, which has the r-slur in it. It is censored in post, and they cut to a judge's face when the lyric comes up, presumably to hide the queens mouthing it.

Prior to season 7 video messages were delivered with a transphobic slur
There is a challenge in episode 2 of season 2 set to a song with repeated use of transphobic slurs
There is an acting challenge in season 3 episode 3 a transphobic slur is used during an acting challenge
Those are the examples I can think of, but earlier seasons really are full of transphobic slurs

In some acting challenges queens are shown "arguing with themselves" as characters with "multiple personalities"
In season 8 episode 3 there's an example of this. I know it's not the only one, but it's the only one I can think of.

In season 11 Yvie Oddly, there are multiple instances of the c slur (cr*pple) when, due to EDS, she uses crutches. They then do the cr*p walk to mock her. Definitely triggered me, as a wheelchair user. There are also abled appropriation of mobility aids in other seasons, and jokes about amputees.

a lot of using "heterosexual" to mean the same thing as "straight".
which implies that if someone was heterosexual and aromantic (for example), that it would be the same as them being straight/not queer at all. Or it's just erasure of aromantics in general.
also, Max from S7 was shamed and made fun of for not liking the sexual aspects the other queens showed. she said "i dont want to rely on grossness or sexuality." and then was portrayed as prudish. Even though we don't know if Max is actually asexual, the shaming of her discomfort may hit a nerve for sex-repulsed asexuals.
