Yes, in the ‘4th incident,’ the antagonist appears to be in a boyfriend-girlfriend relationship. He bullies her (psychologically), tortures her (physically), and then murders her. You DO see him cut off one of her breasts, and it does look very detailed and realistic. At least you did in the version I saw.

In the flashback, watch while the men are cutting grass in the meadows, but pause when they stop to sharpen their scythes. there is a pretty drawn out sequence where the antagonist shows off hunting rifles to two young boys, explains the act of hunting, and eventually forces the children and their mother to flee while he snipes them from a hunting blind. a police officer is shot to death in another scene. in another drawn out sequence, a character taunts a group of kidnapped people about shooting them all through the head with a single bullet. this doesn't end up taking place, but he goes through a lot to setup the rifle and find the right rounds for it.
