Theorizing that one could time travel within his own lifetime, Dr. Sam Beckett stepped into the Quantum Leap accelerator and vanished... He woke to find himself trapped in the past, facing mirror images that were not his own and driven by an unknown force to change history for the better. His only guide on this journey is Al, an observer from his own time, who appears in the form of a hologram that only Sam can see and hear. And so Dr. Beckett finds himself leaping from life to life, striving to put right what once went wrong and hoping each time that his next leap will be the leap home.
The dog dies about 25 minutes into the episode. Cute little guy is killed with his throat slit. I don’t think you see any gore but you see the characters react to it. The woman who owns him is devastated.

5mins into the episode a girl who is auditioning on stage is referred to as someone who gets sick from stage fright. Various gross references are made about it (plus she appears unwell) before she runs off the stage to be sick. I looked away and switched to mute so I'm not sure if there's any visual or audio but I don't think there is. Similar thing begins to happens again around 21mins but then Elvis joins her on stage and she's fine. Al makes a gross reference just before that.
Several times in the series Sam leaps into a woman's body. It is in part due to how the series works rather than a full "man in a dress" joke (but still, it depends of the moment).
There also is (in the first episode where that happens) transmisogyny due to that.
There are numerous instances of hate speech (particularly racist, sexist, ethnic, and ableist slurs), reflective of the time and context in which episodes are set. This language is never glorified or praised, and is frequently challenged by other characters immediately and onscreen.
"A portrait of Troian"- Several people. Though you don't see them drown. It happened in the past and is regularly referenced. Bodies do rise up from the lake right at the end as the earthquakes displaced them.
