Ten years ago, a tragedy changed the town of Harmony forever. Tom Hanniger, an inexperienced coal miner, caused an accident in the tunnels that trapped and killed five men and sent the only survivor, Harry Warden, into a permanent coma. But Harry Warden wanted revenge. Exactly one year later, on Valentine’s Day, he woke up…and brutally murdered twenty-two people with a pickaxe before being killed.

MAJOR SPOILER: Tom Hanniger (Jensen Ackles) is a person suffering from mental health issues due to having survived a killer's attack in a mine. Ten years later he returns. The attacks start happening again, and people think it's the original killer who it turns out actually died 10 years previously. It's discovered that Tom spent 7 years in a mental institution after being suspected of doing the crimes. Tom "turned into" the killer for unknown reasons and appears to be suffering from PTSD and DID. He violently kills a lot of people.
