As punishment for trying to kill Hans, Cleopatra is mutilated so severely that she becomes a “freak” herself, forced to live out the rest of her days as a horrifying sideshow attraction. It’s very disturbing, even though she’s the villain

It's mentioned that the bearded lady gives birth offscreen and characters go to visit her post-birth, but the process of childbirth itself is not shown. Thought it was worth mentioning in case that could be upsetting for anyone.

Lots of disparaging remarks about the “freaks,” such as a man calling them “monsters” due to their disabilities.

The entire movie revolves around the hatred and prejudice against people with disabilities, who are called “freaks” even by the “good guys,” as well as how their only hope at a steady job is to be gawked at in a circus sideshow. While the film sensationalizes the “freaks” and clearly shows off them and their talents (swallowing swords, lighting a cigarette without hands or feet) in the same manner a circus would, the point is that the behavior of the “normal people” Cleopatra and Hercules is far more sinister than anything the “freaks” would normally do. In the end, there is a somewhat muddled anti-ableism message

One of the “freaks” in the movie is a “half man half woman.” Jokes are repeatedly made about what to refer to this person or if they are two people, and the two sides are referred to as “he” and “she” respectively.

Not on screen, but the actors playing Hans and Frieda were siblings in real life and thus their romance scenes had to be toned down significantly.

Good triumphs over evil, and the movie ends with one happy couple and one couple reconciling after almost being torn apart. However, the path to this happy ending was extremely violent and gruesome
