At one point, a bird is shown in a cage, and the solution to that puzzle is to free it. Another scene implies several animals were experimented on.

It's heavily implied an animatronic fortune-teller is haunted, and one room involves performing a seance. You can exorcise the animatronic.

One of the endings of the third game heavily implies that the protagonist was sent to Bedlam, an asylum infamous for mistreating its patients.

There's a perpetual sense of being trapped throughout the games, especially the third game. Several puzzles involves small spaces, especially the fourth game, which takes place in a dollhouse.

Yes and no; the third game features multiple endings. In only one of them do you fully escape the Room, however, and it requires such specific actions that it is difficult to achieve. You will likely get one of the other endings first.

A disembodied human heart and arm are shown, and more than once, a dead body is shown on screen. Little-to-no blood, though.
