Shallan is basically Dissociation the Character. She dissociates from all her negative/traumatic feelings and has developed alternate personalities as a way to dissociate even further. Her alternate selves play a big role in the big climatic fight. This is her main character arc in this book as well as all the other books in the series.

There is a character who is a woman but everyone in a certain context (the military) refers to as a man. Before knowing the context for this was revealed, it felt there could be misgendering going on. It turns out that the character doesn't really mind and this aspect of her character is dropped once they leave the military setting.

There is no sexual assault, but if you're really sensitive like me: there is a description that disturbed me of a war raid in the past where there were many crying women lined up for Sadeus to choose from, but a bargain has been made for their release and they are freed. I believe the implication was Sadeus choosing those to SA considering the pillaging of the war raid. Nothing ultimately occurs, but wanted to give a heads up.
